New Patients

The Georgina Nurse Practitioner-Led Clinic continues to accept applications for new patients. Our clinic provides health care services to residents of the Town of Georgina. We are not a walk-in clinic.

Note: Due to an influx of applications, there is a current waiting time of about 6-12 months. Our team is working very hard to book in new patients while ensuring that current patients’ access to care is not affected. We thank you for your patience as we continue to do our best in accommodating all patient requests. Should you choose to become a clinic patient, you must give up your provider elsewhere.

To become a patient of the Georgina Nurse Practitioner-Led Clinic, please complete the Adult or Child Registration Form below and submit it to the clinic by faxing it to 905-722-3422, dropping it off at the clinic, or calling the clinic to request a secure electronic version of the questionnaire.

The Georgina Nurse Practitioner-Led clinic strives to offer same day or next day appointments for patients who have illnesses that arise suddenly. If you are sick and need to see a Nurse Practitioner today or tomorrow, please call the office.

Prescription renewals and any other non-urgent issues should be by appointment only and arranged in advance. Please book an appointment for renewals one month before your medications run out.

Please bring your health card and medications to every visit. Inform reception of any changes of name, address, phone numbers, pharmacy, and emergency contact.

If calling to book your appointment, please tell the receptionist your reason for the visit. This allows for the appropriate amount of time to be booked.

Cancellations

Please call us 48 hours in advance or as soon as possible to cancel your appointment.

Lab services are available by appointment for registered patients only. Please check in with the receptionist prior to waiting in the lab area.

If you have recently been discharged from the hospital, please call us when you get home to schedule a follow-up visit with your Nurse Practitioner.
